Social Worker - Case Manager

Growing integrated healthcare provider seeks Social Worker - Case Managers as key members to join our patient care team. The Social Worker - Case Manager will assume the role of primary point of contact for patients’ health related needs by facilitating timely access to care, and educating patients and their families on improving overall health. The Social Worker - Case Manager will share patients disease management goals, progress and other pertinent information with the healthcare team to ensure consistency of patient goals, facilitate tracking of patients progress and ensure quality outcomes.  The Social Worker - Case Manager will:
  • Access patient needs and determine priorities for disease management programs and services.
  • Facilitate the review and selection of appropriate patient education resources and tools and strategies for patients.
  • Conduct disease management assessments using standardized instruments and documents in EMR and/or care management platform as appropriate.
  • Act as a resource person for patient, provider, care center staff and the community. 
  • Develop repository of best disease management practices for the disease management team and facilitate educational opportunities for patients. Our client is an integrated healthcare organization providing health plans and comprehensive services to dual eligible (Medicare and Medicaid) patients. Its award winning model is to provide concierge medical services to the most disadvantaged patient population that are typically being poorly served by traditional fee-for-service medical systems.

    Requirements:
  • Licensed Clinical Social Worker - LMSW, LBSW or LLSW in the State of Michigan
  • MS or higher from a CSWE-accredited social work program
  • 3+ years clinical experience, HMO - IPA - Managed Care setting preferred
  • Training in Care Coordination or Case Management; certification preferred
  • Knowledge of community resources.
  • Knowledge of clinical standards of care
  • Knowledge of Medicaid - Medicare contracts and benefit systems is preferred
  • Demonstrated passion for extraordinary patient service and personality for developing close relationships with patients.
  • The ability to travel locally in the greater Detroit area required for home visits, meetings with families, and other regular meetings as well as a weekly regional meetings are required.
  • Knowledge of CPT, ICD-9 and HCPC codes.
  • Professional, flexible, and patient centered “team player” personality
